Death returns to Riau

Over the weekend, 10 year old Gozi Sultia Ningsih died of a confirmed H5N1 infection.  Her 17-year old aunt, Riri Rianti, also died after being hospitalized on October 11, but, incredibly, no samples were taken for analysis.  Just how, in perhaps the most bird flu-ridden province in Indonesia, can no samples be taken of a young girl with suspected H5N1 symptoms?!  This is beyond the imagination of any competent medical professional or layperson. 

This fact is not lost on the central government in Jakarta, which is growing increasingly frustrated with the inability of local medical authorities to observe suspected human H5N1 protocols.  The exasperation is clearly noticeable in the quotes from Dr. H Djuharman Arifinof, chairman of the Dewan Perwakilan Rakyat Daerah (DPRD), or Regional Council of Representatives, representing the Riau district.

What he is saying is that there is a systemic failure to test and report each suspected case of flu burung (avian influenza) and that failure can cost the nation dearly.  I detect frustration not just with the boots on the ground, but also with the central government in Jakarta. 
There is also an uncharacteristic frankness coming from our least favorite Indonesian official, Health Minister Siti Supari, who has, at least today, dropped the Mayor Larry Vaughn approach to H5N1.
JAKARTA, Oct. 22 (Xinhua) -- Indonesian Health Minister Siti Fadilah Supari confirmed Monday that the death of a 10-year-old girl in Riau Province was caused by bird flu, bringing the total death toll to 89.

 

"Yes, it was positive (of bird flu)," she told reporters at the State Palace here.

 

 The girl's aunt had died earlier of suspected bird flu, she added.

 

 "But we are not certain about her aunt because we didn't take her blood sample," the minister said.

 

We now have six confirmed human H5N1 cases, with five deaths.  We also have a strong sneaking suspicion that this should read seven and six, respectively, if not many times more.  We also have another potential family cluster that will go unsolved, due to a lack of due diligence in the field by Indonesian doctors.  And we have a growing political issue in the archipelago, as regional leaders begin to voice their displeasure with both the central government's reaction, as well as with their own physicians.
